- The scope boundary is unclear or in dispute
- Budget authority or approval limits are unclear
- Stakeholder priorities conflict with no obvious resolution path
- A proposed risk mitigation involves systems or teams outside the stated project scope
- Schedule commitments imply resourcing that hasn't been confirmed by the user
